The short answer is: any germs that gets into an nouns that it normally doesn't belong and cause an infection. ie. intestinal bacteria is impressively beneficial for breaking down and helping you digest food but if a few of them get into your bladder or a cut contained by your skin then it is a pathogen. Same entry goes for environmental microbes. ie. the bacteria that cause tetnus doesn't cause any problems within its natural habitat, the soil, but if it get in a cut next you have a problem.
Lastly, at hand are always exceptions to every rule. Staph. aureus and Strep. A (pyogenes) are other pathogens when isolated. Probably some others as well, but to be precise all that I can ponder of at the moment.

There are literally thousands of pathogenic bacteria. All it funds is that that particular germs causes a disease. There are also lots of microbes that are helpful next to processes like digestion, maintain a healthy stability in your body, etc. And greatly of bacteria don't affect us at adjectives.
The diseases of pathogenic bacteria can be as small as an infected cut on your finger, they can affect your lungs approaching pneumonia, they can cause leprosy, etc. You can google it for a more detailed description or you can check out a book at your local library.
